1. I consent to my identity being revealed to the member who is the subject of the complaint.

2. I authorise the Institute of Directors (IoD) to conduct its own enquiries, and to collect and store such information as may be required to investigate and determine the complaint.

3. I agree that the process of investigation and information submitted on behalf of either party remains confidential unless otherwise directed by the IoD’s Council.

4. If a non-member, I agree that the IoD proceed in my capacity as though it were the complainant, as necessary for the investigation and resolution of the complaint.
